Blood Glucose Log Book
The linked document is for MS Word 2010, and provides a simple template for a daily log book for school use. This is what we used for my daughter before switching the school to the Jade Diabetes system.
Print out one page, flip it over top to bottom, and print another copy. Then make 50 double-sided copies, cut in the middle, and you get a simple A5 log book for teachers and support staff to fill in.
